centrelink要求的，获得福利前雇主必须提供离职证明，centrelink会提供那张表格要求雇主填。如果你是contract work或者casual，seasonal的工作填的是另外一张表。

这应该是雇主给你的，Employment Separation Certificate。申请福利时，Centrelink 要求提供。
医生只是开Medical Certificate，或者某些工伤证明之类。

Termination certificate is normally issued by employer, not doctor. Did you tell the doctor you want to quit the job? The doctor might just reminder you that you need a termination certificate if you want to get some benefits from centrelink
