求救，怎么写feedback form

我作it的，刚实现了一个新名目，马上要小规模的piolt test,我的manager让我筹备一个eedback form,
就是提一些问题，让试用的客户回答，要从issues, user experience, document quality三方面筹备问题，你说我又不是project manager, 那会这些阿？请大家指导一二，多谢，回贴必加分

Did you experience any problem with the product?
If Yes, What kind of problem did you experience?
User Experience
What features would you like to see added

Documentation Quality
Suggestions to improve the documents

不是给几个可能的options+other让客户填的么？？
我在读书的时分好像有扫过相干的内容。。不知道有无记错=。=
issues好像是针对你详细软件的功用进行讯问。。
user experience好像是从响应时间，易用性等方面斟酌
documentation quality我印象里那书里没有对应的术语，字面上看是指仿单是不是明晰有条理，便利用户掌握产品。。或者保护方面的document
菜鸟。。纯正帮顶中。。

是的user experience是指零碎performance方面的，好比loading的时间
issue的话倡议你参考一下test case就知道该讯问哪些问题
